Family medicine & primary care

Family medicine is primary care: the first point of contact for most health problems in the community. A psychiatrist learns from it how common mental illnesses usually present and are managed outside hospital, which is where most people with depression and anxiety are seen.

Depth by stage, Pre-U → Consultant (details)

What is it?

Family medicine (primary care) deals with people of all ages and all kinds of problems, often in short consultations at a health clinic (klinik kesihatan) or general practice. It covers chronic disease follow-up, minor illness, prevention, screening, child health and maternal care.

Students learn the consultation model of general practice: listening, managing uncertainty, continuity of care and treating the patient within their family and community. In Malaysia they may be placed in government health clinics and see how the public primary care system works.

Most people with common mental health problems are never seen by a psychiatrist; they are managed in primary care. Understanding this setting helps psychiatrists support and work with family doctors.

Why does a psychiatrist need it?

Depression, anxiety and somatic complaints are among the most common reasons people visit primary care. Psychiatrists need to know what primary care can manage, how to support family doctors and how patients flow between clinic and hospital. Community follow-up of stable patients on psychiatric medication often happens in primary care.

Common questions

Can family doctors treat depression?

Yes. Many cases of mild to moderate depression and anxiety are diagnosed and treated in primary care. Psychiatrists usually see more severe, complex or treatment-resistant cases.

Why does a psychiatrist need to understand primary care?

Because most mental health care happens there. Psychiatrists who understand primary care can give practical advice, design sensible referral systems and share care of stable patients.
